The service aims to support eligible participants to remain living in the family home by offering case management and or discretionary funding.
People with a disability aged 6-65 who are eligible under the Disability Act 2006.
Making a Difference provides short term case management and/or discretionary funding.
Intensive support case management is available if required.
Aged between 6 and 64 years.
To be eligible under the Disability Act 2006 the person must have a significant sensory, physical or neurological impairment or acquired brain injury or an intellectual disability and have a permanent disability that significantly impacts upon and limits the person’s day to day functioning and community involvement.
There is no charge for this service.
Anyone can make a referral as long as the family has given its consent.
Referrals are lodged through the Connecting Care website.
