Individual Support Plan

Aim

The service aims to facilitate life planning for a person with a disability.

Participants

People aged 6-64 with disabilities who are eligible under the Disability Act 2006.

Description

We help the participant to identify their short and long term goals and help with the implementation of the plan by developing a disability service request for funds where necessary.

Aged between 6 and 64 years.

Accessing a disability service

To be eligible under the Disability Act 2006 the person must have a significant sensory, physical or neurological impairment or acquired brain injury or an intellectual disability and have a permanent disability that significantly impacts upon and limits the person’s day to day functioning and community involvement.

Fees

There is no charge for this service.

Referral Information

Anyone can make a referral as long as the family has given its consent.

Referrals are lodged through the Connecting Care website.
